I authorize Unique Card Services to charge my credit/debit card immediately for the initial application fee of $29.95 and every (30) days thereafter $19.95 in which I maintain my membership. The charge will appear as Unique Card Services. My merchandise credit line can be used exclusively at www.myuniqueoutlet.com. I may cancel my membership at any time by calling 1-888-805-7648 or at memberservices@uniquecardservices.com. If I cancel my membership after the initial or monthly debit has occurred, I will cancel all subsequent debits, but will not receive a refund for the prior or current months if I am outside my initial (30) day cancellation period.
